What is epilepsy?

Epilepsy is a non-communicable chronic brain disorder that causes recurring, unprovoked seizures. It is also known as a seizure disorder.

The condition refers to episodes of involuntary movements of many parts of the body. Sometimes accompanied by unconsciousness and uncontrolled bowel or bladder function.

Note that having one seizure doesn’t mean epilepsy. Epilepsy means having at least two unprovoked seizures at least 24 hours apart.

The condition can affect people of all ages, genders, and ethnic backgrounds. As per data, low- and middle-income countries register the most cases.

Its symptoms can vary widely. It can be the briefest lapses of attention, muscle jerks or a prolonged convulsion. The frequency of seizures can also vary, from less than one per year to several per day.

Types of epilepsies and their symptoms

Focal-onset seizures (partial-onset seizures)

This type of seizure starts in one area or on one side of the brain. A focal seizure has two types:

  • Focal onset aware seizure (simple partial seizure)
  1. Awake and aware during the seizure
  2. Emotional changes
  3. Experience changes in taste, smell or sound
  4. Uncontrolled muscle jerking, usually in the legs or arms
  5. Dizziness
  6. Tingling sensation
  7. Seeing flashy lights
  • Focal-onset impaired awareness seizure (complex partial seizure)
  1. Loss of awareness or consciousness
  2. Blank stare
  3. Repetitive movements like lip-smacking, hand rubbing or eye-blinking

Generalised-onset seizures

This type of epilepsy affects both sides of the brain at the same time. It has six types:

  • Absence seizure
  1. Blank stare
  2. The seizure lasts for a few seconds
  3. Minor muscle movements (eye blinking, lip smacking, or hand motions)
  • Atonic seizure
  1. Loss of muscle control
  2. Weakness in muscles
  3. Fall to the ground
  4. The seizure lasts for a few seconds
  5. Parts of the body, like the eyelids or head, may droop or drop
  • Tonic seizure
  1. Awake or experience a little change in awareness
  2. Increase in muscle tone
  3. Stiffness in the back, arms, legs or whole body
  4. Fall to the ground
  5. Lasts for a few seconds
  • Clonic seizures
  1. Continuous, fast muscle jerking or muscle stiffness followed by jerking
  2. It lasts for a few seconds to a few minutes
  • Tonic-clonic seizures
  1. Combination of tonic and clonic seizures
  2. Muscle stiffness and continuous, fast muscle jerking
  3. Loss of consciousness
  4. Fall to the ground
  5. Muscle stiffness and jerking for a few minutes
  6. May bite the tongue or drool
  7. Uncontrolled bladder or bowels function, making the affected person pee or poop
  • Myoclonic seizures
  1. Brief, shock-like jerks or twitches
  2. Lasts for a few minutes

Diagnosis

Epilepsy diagnosis is usually done by an electroencephalogram (EEG) and a brain scan. Sometimes doctors may recommend other tests as well.
